Pouring concrete is more complicated than it looks.

For example, when you see a concrete delivery truck on the road with the barrel turning, depending on the PSI, the barrel must turn a certain number of revolutions per minute, or the concrete will start to get hot and eventually harden in the truck. If the concrete contractor is not ready for the pour and there are too many revolutions, the concrete will get too hot, and within a week of the pour, it will start to have cracking throughout the slab. Other problems occur when concrete is finished too soon or too late, and spider cracking will occur (this is called crazing).

Segregation is a situation where the components of the concrete (sand, aggregate, rocks-stones, Portland cement) start to separate because of not being properly handled, transported, or placed, and the entire pour must be demolished, discarded, and re-poured.
